Job summary

Concierge Home Care in Pensacola, FL is seeking a Home Health Speech-Language Pathologist (SLP) for flexible, PRN work with pay-per-visit opportunities.

You will conduct assessments, develop plans, and deliver 1:1 therapy in patients' homes within Pensacola/Escambia County, with support and ongoing training. Excellent autonomy and income potential.

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Valid drivers license and reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using AI-assisted technology.
