Job summary

Concierge Home Care in Sarasota, FL is seeking a Speech-Language Pathologist to provide skilled services in patients' homes, helping with communication, cognition, and swallowing. The role emphasizes autonomy, flexible scheduling (PRN and contractor options), and a pay-per-visit model designed to reward productivity.

You will use AI-assisted charting and WellSky/Kinnser EMR to document visits efficiently, participate in onboarding and ongoing clinical training, and collaborate with physicians

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Valid driver's license and reliable transportation.
• Home health experience preferred but not required.
• Medicare OASIS documentation experience preferred.

Responsibilities

• Conduct comprehensive speech, language, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy focused on communication, cognition, and swallowing disorders.
• Educate patients and caregivers on techniques to improve functional outcomes.
• Collaborate with physicians and interdisciplinary care teams.
• Complete Medicare OASIS documentation as required.
• Document visits efficiently using AI-assisted tools.

As a Home Health SLP, you will provide skilled speech therapy services in the comfort of patients’ homes while helping them improve communication, cognition, and swallowing function.
