Concierge Home Care in Lake City, FL is seeking a licensed Speech-Language Pathologist to provide home health services on a PRN or flexible pay-per-visit basis. You will deliver 1:1 patient care, perform evaluations, and develop treatment plans according to physician orders.

Qualifications

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
