Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) to provide patient-centered care across Jackson, Calhoun, Liberty, Washington, and Holmes counties from Marianna, FL. The role offers flexible scheduling and autonomous practice with guaranteed 1:1 patient contact in the home setting.

Typical duties include SLP evaluations, personalized treatment planning, dysphagia therapy, and collaboration with an interdisciplinary team.

Qualifications

• Active Florida SLP license
• Valid driver's license and reliable transportation
• Home health experience preferred
• OASIS/Medicare documentation experience preferred

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans based on physician orders
• Provide therapy for communication, cognition, and dysphagia
• Educate patients and caregivers to improve functional outcomes
• Collaborate with physicians and interdisciplinary teams
• Complete OASIS documentation as required
• Document visits using AI-assisted technology

Your Role as a Home Health Speech-Language Pathologist
