Job summary

Concierge Home Care in Orange Park, FL is seeking a Home Health Speech-Language Pathologist (SLP) for PRN, flexible scheduling, and pay-per-visit compensation. Coverage area includes Orange Park and Clay County with clustered scheduling to minimize drive time.

You will perform 1:1 in-home therapy, conduct evaluations, and develop individualized plans. AI-assisted charting and WellSky/Kinnser EMR support time and documentation.

Qualifications

• Active Florida Speech-Language Pathologist license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition and dysphagia.
• Educate patients and caregivers to improve outcomes.
• Document visits using streamlined AI-assisted technology.

Your Role as a Home Health Speech-Language Pathologist
