Concierge Home Care in Fort Walton Beach, FL is seeking a Home Health Speech-Language Pathologist (SLP) for flexible, patient-centered home health therapy. You will conduct evaluations, create individualized plans, and deliver therapy focused on communication, cognition, and swallowing.

Use AI-assisted charting and WellSky/Kinnser EMR to streamline documentation. Enjoy autonomy with a PRN/1099 arrangement, mileage reimbursement, and a $1,000 referral bonus per full-time hire.

Qualifications

• Active Florida SLP license
• Valid driver’s license and reliable transportation
• Home health experience preferred but not required
• OASIS/Medicare documentation experience preferred

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans based on physician orders
• Provide therapy for communication, cognition, and dysphagia
• Educate patients and caregivers to improve functional outcomes
• Collaborate with physicians and interdisciplinary teams
• Complete OASIS documentation as required
• Document visits using AI-assisted technology
