Job summary

Concierge Home Care in Pensacola, FL is seeking a Home Health Speech-Language Pathologist (SLP) to deliver 1:1 in-home evaluations and therapy with flexible, pay-per-visit compensation and autonomy.

You’ll conduct assessments, develop individualized treatment plans, and collaborate with physicians, using AI-assisted charting and WellSky/Kinnser EMR to streamline documentation, while pursuing professional growth and CEUs.

Qualifications

• Active, unencumbered Florida SLP license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
