Job summary

Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) to provide 1:1 in-home therapy across the Ocoee/Orange County area. Enjoy flexibility, autonomy, and meaningful patient relationships while controlling your income with a per-visit pay model and Medicare differential earnings.

You will conduct comprehensive speech, language, cognitive, and swallowing assessments, develop physician-ordered treatment plans, and collaborate with physicians and the care team.

Qualifications

• Active Florida Speech-Language Pathologist license.
• Valid driver's license with reliable transportation and auto insurance.
• OASIS/Medicare documentation experience preferred.
• Home health experience preferred but not required.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
