Job summary

Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) to serve the Jackson, Calhoun, Liberty, Washington, and Holmes Counties area in Florida. Manage your own schedule with PRN, or 1099 contractor options, delivering 1:1 home-based care with autonomy and strong income potential.

Qualifications

• Active Florida SLP license required.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.

Your Role as a Home Health Speech-Language Pathologist

Qualifications

• Valid driver’s license, reliable transportation, and auto insurance
• Home health experience preferred but not required
