Job summary

Concierge Home Care is seeking a Home Health Speech-Language Pathologist (SLP) to join our Fort Walton Beach team on a PRN/1099 basis. Enjoy a flexible schedule with true 1:1 patient care in the home, no weekends or hospital shifts, and autonomy to manage your own caseload across Fort Walton Beach and Okaloosa County.

Earn $75+ per home visit, with additional compensation for Medicare differentials. You'll conduct evaluations, develop individualized treatment plans, document visits with

Qualifications

• Active, unencumbered Florida SLP license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
