Job summary

Concierge Home Care is hiring a Home Health Speech-Language Pathologist (SLP) to provide 1:1 patient care in the Fort Walton Beach area. This PRN, pay-per-visit role offers flexible scheduling, autonomy, and potential for increased earnings through Medicare differentials.

You will conduct evaluations, develop treatment plans per physician orders, and document visits using AI-assisted charting and WellSky/Kinnser EMR.

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using AI-assisted technology.
