Job summary

Concierge Home Care is a Home Health agency hiring a Speech-Language Pathologist (SLP) for PRN, pay-per-visit work in Lake City, FL. Enjoy a flexible schedule, autonomy, and true 1:1 patient care in the home.

You will conduct evaluations, develop treatment plans, educate patients and caregivers, and complete OASIS documentation while collaborating with the care team. Employment is contingent upon AHCA Level 2 background screening.

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using AI-assisted technology.
