Job description

BAYADA Home Health Care

is currently seeking an experienced Speech Language Pathologist, SLP for a per diem opportunity performing home health visits for our senior living office located in Charlotte, NC.

• This office services adult and geriatric patients in senior communities throughout Mecklenburg County.

• Prior home care experience as a Speech Language Pathologist, SLP, is a plus but not required.

As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive the medical care needed to remain at home.

Speech Language Pathologist, SLP Responsibilities:

1. Make home visits to clients in designated geographic territories.
2. Perform evaluations to assess, diagnose, and provide therapeutic interventions for clients experiencing speech, language, communication, or swallowing difficulties.
3. Develop a treatment plan based on evaluations, physician's orders, and goals.
4. Assess and revise the speech therapy care plan regularly, collaborating with BAYADA clinicians as appropriate.
5. Educate and instruct patients, family members, or other representatives in rehabilitative care to promote health, safety, and independence.
6. Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.

Speech Language Pathologist, SLP Qualifications:

1. Current license as a Speech Language Pathologist in North Carolina.
2. Certificate of clinical competence in speech from the American Speech and Hearing Association, or equivalent educational and work experience, with pending certification examination.
3. Solid computer skills; prior experience with electronic medical records (EMR) preferred.
