Job description

BAYADA Home Health Care

is currently seeking a Speech Language Pathologist (SLP) for a per diem opportunity performing senior living visits.

This office services adult and geriatric patients in senior living communities throughout Lincolnton, NC.

As a home care Speech Language Pathologist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive the medical care required to remain at home.

Speech Language Pathologist, SLP Responsibilities:

1. Make home visits to clients in designated geographic territories.
2. Perform evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
3. Develop a treatment plan based on evaluation, physician's orders, and goals.
4. Assess and revise the speech therapy care plan, participating in multidisciplinary care planning.
5. Educate patients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a touch pad tablet.

Qualifications:

1. Current license as a Speech Language Pathologist in NC.
2. Certificate of clinical competence in speech from the American Speech and Hearing Association, or equivalent educational and work experience, with eligibility to take the ASHA examination.
3. Solid computer skills; prior experience with electronic medical records (EMR) preferred.
