Job summary

Empath Home Health seeks a compassionate PRN Speech Language Pathologist to provide in-home SLP services in Naples, FL and surrounding areas. The role involves performing assessments, developing plans of care, and educating patients and families on swallowing, language, and cognitive strategies.

The ideal candidate has CCC-SLP credentials, an active Florida license, and at least 1 year of SLP experience, preferably in home health.

Qualifications

• Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology granted by the ASHA; or enrolled in a program that meets the educational requirements for certification and accruing supervised experience.
• Active Florida SLP license.
• Minimum of 1 year of experience as an SLP; home health experience preferred.
• Strong clinical assessment, communication, and patient education skills.

Responsibilities

• Perform initial and ongoing SLP assessments in patients' homes, coordinating care with the physician and care team.
• Develop and implement plans of care for swallowing, cognition, and communication.
• Educate patients and families on therapeutic strategies and home exercises.
• Document services and collaborate with the interdisciplinary team for holistic care.
