Empath Health is seeking a compassionate PRN Speech Language Pathologist to serve patients in Port Charlotte & North Port, FL, providing in-home evaluations and therapy across a flexible schedule. The SLP will collaborate with a multidisciplinary team, address swallowing and communication disorders, and document progress in the Homecare Homebase EMR.

The role requires an active Florida SLP license (or eligibility) and experience in home health preferred.

Qualifications

• Active Florida SLP license or eligible for licensure.
• Program that meets ASHA CCC requirements or in progress toward CCC.
• Experience in home health settings preferred.

Responsabilités

• Perform initial and ongoing assessments including SLP evaluations in home health settings.
• Develop and implement plans of care with interdisciplinary team.
• Educate patients and families on swallowing, communication strategies and cognitive-linguistic therapy.
• Document services and progress notes in a timely manner.
• Collaborate with physicians to ensure coordinated care.
