Job summary

Empath Home Health, a member of Empath Health, seeks a compassionate PRN Speech Language Pathologist to serve patients in Naples, FL and surrounding areas.

As part of our Home Health team, you will perform assessments, develop plans of care, and educate patients and families on swallowing safety and communication strategies, collaborating with the interdisciplinary team for holistic care.

Qualifications

• Graduate of an accredited program meeting ASHA certification requirements or in process toward certification.
• Active Florida SLP license.
• Minimum of 1 year of licensed SLP experience; home health preferred.
• Strong clinical assessment, communication, and patient-education skills.

Responsibilities

• Perform initial and ongoing assessments, including OASIS when appropriate.
• Develop a care plan in collaboration with the physician and care team.
• Educate patients and families on swallowing safety and compensatory strategies.
• Provide evidence-based communication/intervention to improve language and cognition.
