Job summary

Centra Healthcare Solutions, Inc. seeks an experienced Speech Language Pathologist (SLP) for a contract assignment in Modesto, CA.

The role focuses on delivering skilled therapy services in SNF/LTC, collaborating with an interdisciplinary team to implement physician orders and care plans. Candidates must be a graduate from an accredited program, hold or be pursuing state licensure as an SLP/CCC, and be able to set realistic goals for communication and swallowing in daily practice.

Qualifications

• Must have graduated from an accredited school.
• Current state license as SLP or in licensure process.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services per physician orders.
• Set realistic goals to maintain/recover communication and swallowing abilities.

A great SNF / LTC is looking for an experienced Speech Language Pathologist / SLP to join their growing team for a contract assignment!

This is a contract position.

Days schedule.

This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or proactively in the process of the application process for current state licensure as a Speech Language Pathologist / SLP.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills so that the patient can fulfill their educational, vocational and social rolesCandidate must be an SLP-CCC.
