Centra Healthcare Solutions is seeking an experienced Speech Language Pathologist (SLP-CCC) for a contract assignment at a SNF/LTC setting in the United States. The role requires providing skilled speech therapy services, assessments, and goal-driven plans of care under physician orders, within an interdisciplinary team.

Candidates must be licensed or in process, hold CPR, and graduated from an accredited program.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or in process for licensure.
• Current CPR certification.

Responsibilities

• Provide skilled speech therapy services/interventions per physician orders.
• Assess patient needs and plan, evaluate, and modify care for speech therapy goals.
• Set realistic goals for patients to develop or recover communication and swallowing skills.
• Work within an interdisciplinary team to develop a plan of care for each patient.

A great SNF / LTC is looking for an experienced Speech Language Pathologist / SLP to join their growing team for a contract assignment! This is a contract position.

Days schedule.

This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or proactively in the process of the application process for current state licensure as a Speech Language Pathologist / SLP.
• Current CPR certification.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills so that the patient can fulfill their educational, vocational and social rolesCandidate must be an SLP-CCC.
