Job summary

Centra Healthcare Solutions Inc is seeking an experienced Speech Language Pathologist / SLP for a contract assignment in Pueblo, Colorado. This role focuses on providing skilled speech therapy services in SNF/LTC settings, following physician orders and collaborating with an interdisciplinary team.

The ideal candidate holds SLP-CCC, licensure in the state, CPR certification, and a commitment to achieving goals in communication and swallowing for patients.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or in process of licensure.
• Current CPR certification.
• SLP-CCC certification required.

Responsibilities

• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assess patient needs, plan for, evaluate and modify care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop or recover communication and swallowing skills.

A great SNF / LTC is looking for an experienced Speech Language Pathologist / SLP to join their growing team for a contract assignment! This is a contract position. Days schedule. This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or proactively in the process of the application process for current state licensure as a Speech Language Pathologist / SLP.
• Current CPR certification.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills so that the patient can fulfill their educational, vocational and social roles.
• Candidate must be an SLP-CCC.
