Job summary

Centra Healthcare Solutions Inc is seeking an experienced Speech Language Pathologist (SLP) for a contract assignment in a SNF/LTC setting. The role involves working with an interdisciplinary team to provide skilled speech therapy services, assess needs, plan goals, and follow physician orders.

Requirements include graduation from an accredited program, current state licensure or in process, CPR certification, and SLP-CCC.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or in the process of licensure.
• Current CPR certification.
• Candidate must be an SLP-CCC.

Responsibilities

• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of speech therapy interventions.
• Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills.
