Job summary

Valley Oaks Post Acute in California is seeking a full-time Speech-Language Pathologist (SLP) to join our in-house rehabilitation team. You will evaluate and treat residents, develop individualized plans, and support communication, cognition, swallowing, and independence.

Qualifications

• Active Speech-Language Pathology state license in good standing.
• Current ASHA certification.
• License in good standing with applicable boards/agencies.
• Skilled nursing or long-term care experience preferred.
• Knowledge of Net Health Electronic Documentation System preferred.
• Strong communication and clinical skills.

Responsibilities

• Evaluate and treat speech, language, cognitive, and swallowing disorders
• Develop and implement individualized treatment plans
• Educate residents, families, caregivers, and clinical staff
• Collaborate on diagnostic swallowing procedures, including MBSS and FEES
• Maintain accurate, timely, and compliant documentation
• Communicate changes in resident conditions to the care team
• Support restorative nursing and interdisciplinary care initiatives
• Ensure compliance with all professional, legal, and confidentiality standards
