Job summary

Pacific Hills Post-Acute is seeking a Speech-Language Pathologist (SLP) to assess, treat, and support residents with speech, language, cognitive communication, and swallowing disorders. This role improves residents’ communication, safety, and quality of life in a collaborative rehabilitation setting.

Responsibilities include evaluations, individualized therapy plans, accurate documentation, and PDPM participation. Candidates may include CFY; California license required; Chicago?

Qualifications

  • Active California Speech-Language Pathologist license in good standing.
  • Master's or Doctorate degree in Speech-Language Pathology.
  • CFY candidates welcome; experience in skilled nursing or rehabilitation settings preferred.
  • Knowledge of PDPM and Medicare in skilled nursing facilities.

Responsibilities

  • Conduct comprehensive speech-language and swallowing evaluations.
  • Develop and implement individualized therapy treatment plans.
  • Document therapy sessions and resident progress accurately and timely.
  • Collaborate with the interdisciplinary team and residents’ families.
  • Participate in care planning and PDPM processes.
  • Mentor and support Speech-Language Pathology assistants, as applicable.
