Job summary

Homeside Rehab is seeking a compassionate Speech-Language Pathologist (SLP) to provide home-based therapy on Long Island, NY. This part-time/per diem role offers flexible scheduling and the opportunity to add cases to your existing schedule while delivering one-on-one care in patients' homes.

You will work with patients in West Babylon and Far Rockaway, NY, performing therapy per individualized plans and maintaining thorough documentation.

Qualifications

• Active New York State Speech-Language Pathologist license.
• Strong clinical and interpersonal skills.
• Reliable with ability to maintain consistent visits.
• Basic computer skills for electronic documentation.
• Home-based therapy experience preferred but not required.

Responsibilities

• Provide skilled speech-language therapy based on each patient’s individualized plan of care.
• Implement speech, language, cognitive and swallowing interventions as indicated.
• Monitor patient progress and communicate changes as needed.
• Maintain accurate and timely clinical documentation.

Rehab is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to provide home-based therapy services in Long Island.

This is a flexible opportunity for an SLP looking to add cases to their existing schedule while providing meaningful one-on-one care in the comfort of patients’ homes.

Job Responsibilities

• Provide skilled speech-language therapy based on each patient’s individualized plan of care

• Implement appropriate speech, language, communication, cognitive, and swallowing interventions as clinically indicated

• Monitor patient progress and communicate changes as needed

• Maintain accurate and timely clinical documentation

• Communicate professionally with patients, families, caregivers, and the clinical team

• Follow all applicable clinical and documentation requirements
