Job summary

Homeside Rehab is seeking a compassionate Speech-Language Pathologist (SLP) to join our pediatric home health team in Syracuse, NY. In this role, you will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility and a supportive team environment.

Active NYS SLP license required; minimum 1 year of home health, outpatient, or SNF experience preferred.

Qualifications

• Active New York State Speech-Language Pathologist license.
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings preferred.
• Basic computer literacy for documentation and communication.
• Strong clinical and interpersonal communication skills.
• Ability to work independently in a home health environment.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on patients’ needs and goals.
• Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders.
• Maintain accurate and timely documentation of patient care.
• Collaborate with the clinical team to ensure coordinated and effective treatment.
• Communicate effectively with office staff, patients, and caregivers.
• Collaborate with healthcare professionals to ensure comprehensive care.

Homeside Rehab is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our pediatric home health team in Syracuse, NY. In this role, you will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility and a supportive team environment.

Requirements

• Active New York State Speech-Language Pathologist License — required
• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred
• Basic computer literacy for documentation and communication
• Strong clinical and interpersonal communication skills
• Ability to work independently in a home health environment
• Reliable transportation for home visits preferred
