Job summary

Homeside Rehab is seeking a licensed Speech-Language Pathologist (SLP) for home care opportunities on Long Island, NY. The role focuses on evaluating patients, developing individualized treatment plans, and delivering one-on-one therapy in patients' homes.

Flexible scheduling, day and evening availability, and a mix of full-time, part-time, and per diem opportunities are offered across multiple locations including Mastic Beach and Far Rockaway. A compassionate, collaborative clinician is ideal.

Qualifications

• Active New York State Speech-Language Pathologist license is required.
• Experience in home health care is preferred.
• Excellent communication and interpersonal skills are essential.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled speech-language therapy based on each patient’s needs and goals.
• Monitor patient progress and adjust treatment plans as appropriate.
• Complete accurate and timely documentation.

Homeside Rehab is seeking a licensed Speech-Language Pathologist (SLP) for home care opportunities in Long Island, NY.

We are looking for compassionate and dependable clinicians who are passionate about providing high-quality, one-on-one care while helping patients improve communication, speech, language, cognition, and swallowing skills in the comfort of their homes.

Responsibilities

• Evaluate patients and develop individualized treatment plans

• Provide skilled speech-language therapy based on each patient’s needs and goals

• Monitor patient progress and adjust treatment plans as appropriate

• Complete accurate and timely documentation

• Communicate effectively with office staff, patients, and caregivers

• Collaborate with the clinical team to ensure coordinated and effective treatment

• Work with healthcare professionals, including speech therapy assistants, nurses, physicians, and other members of the care team, to support comprehensive patient care
