Job summary

Homeside Rehab is seeking compassionate Speech-Language Pathologists to provide individualized, evidence-based therapy in patients' homes across New York City's five boroughs. You will evaluate, design treatment plans, and deliver care while collaborating with the healthcare team for coordinated patient progress.

Licensure in New York State is required; CCC-SLP is preferred. Minimum 1 year in home health or related settings is desirable.

Qualifications

• Active New York State Speech-Language Pathologist License (Required)
• CCC-SLP certification preferred
• Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or SNF preferred
• Excellent clinical assessment and communication skills
• Ability to work independently in a home health setting
• Basic computer proficiency for electronic documentation
• Reliable transportation for home visits preferred

Responsibilities

• Perform comprehensive speech-language evaluations and develop individualized treatment plans based on each patient's needs and goals.
• Provide evidence-based therapy for speech, language, articulation, fluency, voice, feeding, and swallowing disorders, when applicable.
• Deliver skilled speech therapy services in patients' homes while promoting functional communication and overall progress.
• Complete accurate, timely, and compliant electronic documentation.
• Collaborate with physicians, caregivers, teachers, and the interdisciplinary healthcare team to coordinate patient care.
• Educate families and caregivers on home programs and strategies to support continued progress.
• Communicate effectively with office staff regarding scheduling, patient updates, and clinical needs.

Position Details

• Compensation: Competitive hourly pay (based on experience)

• Schedule:
