Job summary

The Phoenix Center for Rehabilitation and Nursing in Phoenixville, PA is seeking a Speech Language Pathologist to join our therapy team as a Per Diem/Full-Time employee. New grads welcome and supported by an experienced team.

You will provide rehabilitative therapy for communication, cognitive and swallowing disorders, with documentation of progress and outcomes to support care decisions.

Qualifications

• Must be graduate of an accredited program in Speech-Language Pathology.
• Must be ASHA certified (CCCs).
• Must have current and active license with the state of Pennsylvania.
• Three years experience holding active CCCs prior to CFY Supervision preferred.

Responsibilities

• Provide rehabilitative therapy to patients with communication, cognitive and swallowing disorders.
• Document objective measures and justification for continued or discontinued services.
• Collaborate with the Director of Rehabilitation Services to plan care.

Job Description

The Speech Language Pathologist is responsible for providing rehabilitative therapy to patients upon referral for communication, cognitive and/or swallowing disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services.

This position reports to the Director of Rehabilitation Services.

Job Requirements
