Job summary

Desert Peak Care Center in Phoenix, AZ is seeking a Speech Language Pathologist to evaluate, diagnose, and treat residents with communication, cognitive-linguistic, and swallowing disorders. You will work closely with an interdisciplinary team to help residents regain independence and improve their quality of life.

The role involves developing individualized plans, delivering therapy, educating families, and documenting progress in accordance with regulations.

Qualifications

  • Master’s degree in Speech-Language Pathology from an accredited program.
  • Current Arizona licensure or eligibility to obtain.
  • CCC-SLP certification preferred.
  • Current CPR certification (or willingness to obtain).
  • Experience in a skilled nursing facility or with geriatric populations preferred.

Responsibilities

  • Conduct evaluations and develop treatment plans for speech, language, cognitive, and swallowing disorders.
  • Provide direct therapy services per the plan of care.
  • Collaborate with physicians, nurses, and rehabilitation staff for comprehensive resident care.
  • Educate residents, families, and staff on communication strategies and safe swallowing techniques.
  • Document evaluations, treatments, and progress per state, federal, and facility regulations.
  • Participate in care planning, discharge planning, and quality improvement activities.
  • Provide training and supervision to Speech-Language Pathology Assistants (if applicable).

Position Summary:

As an SLP, you will evaluate, diagnose, and treat residents with communication, cognitive-linguistic, and swallowing disorders. You will work closely with an interdisciplinary team to help residents regain their independence and improve their quality of life.

Key Responsibilities:
  • Conduct evaluations and develop individualized treatment plans for residents with speech, language, cognitive, and swallowing disorders.
  • Provide direct therapy services in accordance with the plan of care.
  • Collaborate with physicians, nurses, and other rehabilitation professionals to ensure comprehensive resident care.
  • Educate residents, families, and staff on communication strategies and safe swallowing techniques.
  • Document all evaluations, treatments, and resident progress in compliance with state, federal, and facility regulations.
  • Participate in care planning meetings, discharge planning, and quality improvement activities as needed.
  • Provide training and supervision to Speech-Language Pathology Assistants (if applicable).

  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred;
