Homeside Rehab is seeking a licensed Speech-Language Pathologist (SLP) for home care opportunities in Newburgh, NY 12550. This position offers flexible scheduling, one-on-one patient care, and the opportunity to add home care cases to your current schedule.

The role focuses on evaluating and treating patients in their homes, developing individualized plans, and collaborating with a supportive clinical team while maintaining thorough documentation and communication with patients and caregivers.

Qualifications

• Active NYS SLP license required.
• Strong communication and clinical skills.
• Ability to work independently.
• Reliable transportation for home visits.
• Home health, outpatient, or SNF experience preferred.

Responsibilities

• Provide skilled speech-language therapy in the patient’s home.
• Evaluate patients and develop individualized treatment plans.
• Provide treatment based on the patient’s speech, language, communication, and swallowing needs.
• Monitor patient progress and adjust treatment as appropriate.
• Complete accurate and timely documentation.
• Communicate with patients, caregivers, office staff, and other members of the clinical team.
