Job summary

Homeside Rehab, Pt, Ot, Slp LLC is seeking a Speech-Language Pathologist (SLP) to provide home-based therapy services on Long Island. This part-time/per diem role offers flexible scheduling and a sign-on bonus, with opportunities to take on additional cases around your existing schedule.

The position is based in West Babylon, NY 11704 and Far Rockaway, NY 11691, with a compassionate clinical team to support flexible, patient-centered care in patients' homes.

Qualifications

• Active NY SLP license.
• Strong clinical and interpersonal skills.
• Reliable and able to maintain consistent scheduled visits.
• Basic computer skills for electronic documentation.
• Previous home-based therapy experience preferred, but not required.

Responsibilities

• Provide skilled speech-language therapy based on each patient’s individualized plan of care.
• Implement appropriate speech, language, communication, cognitive, and swallowing interventions as clinically indicated.
• Monitor patient progress and communicate changes as needed.
• Maintain accurate and timely clinical documentation.
• Communicate professionally with patients, families, caregivers, and the clinical team.
• Follow all applicable clinical and documentation requirements.

Speech-Language Pathologist (SLP)

Rehab is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to provide home-based therapy services in Long Island.

This is a flexible opportunity for an SLP looking to add cases to their existing schedule while providing meaningful one-on-one care in the comfort of patients’ homes.

Job Responsibilities
