Job summary

Homeside Rehab is seeking compassionate Speech-Language Pathologists (SLPs) to provide individualized, evidence-based speech therapy services to patients in their homes throughout the Hudson Valley, New York.

Join a supportive clinical team with a flexible schedule and opportunities to work with patients on communication, language, cognitive-communication, feeding, and swallowing goals.

Qualifications

• Active NYS Speech-Language Pathologist license required.
• Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or SNF preferred.
• Excellent clinical assessment and communication skills.
• Ability to work independently in a home health setting.
• Valid New York State driver's license and reliable transportation preferred.

Responsibilities

• Perform comprehensive speech-language evaluations and develop individualized treatment plans based on each patient's needs and goals.
• Provide evidence-based treatment for speech, language, articulation, fluency, voice, cognitive-communication, and swallowing disorders, when applicable.
• Deliver skilled speech therapy services in patients' homes while promoting improved communication and functional independence.
• Complete accurate, timely, and compliant electronic documentation.
• Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care.
• Educate patients and caregivers on home programs and communication strategies.
• Communicate effectively with office staff regarding scheduling, patient progress, and clinical needs.

Homeside Rehab is seeking compassionate and dedicated Speech-Language Pathologists (SLPs) to provide individualized, evidence-based speech therapy services to patients in their homes throughout the Hudson Valley, New York.
