Responsibilities

• Direct comprehensive speech therapy evaluation and treatment planning.
• Implement therapy plans for swallowing and communication disorders.
• Collaborate with healthcare team to coordinate patient care.
• Document progress and maintain patient records per policy.
• Ensure compliance with Medicare/Medicaid reimbursement requirements.
• Provide patient and family education on therapy goals.
• Deliver therapy in SNF settings with standard modalities.
• Participate in quality assurance and case reviews.
