Job summary

Bremerton Trails Post Acute is seeking a licensed Speech-Language Pathologist to lead patient evaluations and therapy in a long-term care setting. The role involves developing treatment plans, educating families and staff, and ensuring documentation supports patient outcomes.

The position requires current licensure, competency in English, and the ability to manage multiple tasks while adhering to safety and regulatory standards.

Qualifications

• Must hold current, active license as Speech Pathologist and be able to practice unencumbered.
• Graduate of an approved Speech-Language Pathology curriculum; knowledgeable about regulatory requirements.
• Excellent verbal and written communication for patient/family education, documentation, and collaboration with care teams.

Responsibilities

• Conduct evaluations and develop treatment plans per M.D. orders and facility guidelines.
• Provide therapy and document progress, ensuring reimbursement alignment and best functional outcomes.
• Educate staff, patients, families, and payers on therapy benefits and outcomes.
• Complete timely documentation including evaluations, progress notes, and monthly summaries.
• Screen individuals for potential speech-language pathology intervention and coordinate with rehab team.
