Job summary

Synchrony Rehab, a subsidiary of Trilogy Health Services LLC, is seeking a licensed Speech Language Pathologist to join our interdisciplinary team in Wisconsin. You must hold an active or pending SLP license to apply.

This role focuses on assessment, treatment planning, and therapeutic interventions in a collaborative environment. The ideal candidate has a Master's degree in Speech Language Pathology, 0-1 years of experience, and familiarity with CCC-SLP requirements.

Qualifications

• Master's degree in Speech Language Pathology from an accredited program.
• Active or pending state SLP licensure.
• CCC-SLP from ASHA preferred or required.
• Current CPR certification preferred

Responsibilities

• Evaluate patients to identify speech, language, voice, and swallowing disorders through clinical assessments, standardized tests, and patient interviews.
• Develop and implement individualized therapy plans based on assessment findings, setting measurable goals.
• Provide therapy to improve patients' communication skills, including articulation, language, voice, and swallowing.
• Educate patients and families about the nature of disorders and therapy strategies to support progress.
• Maintain accurate documentation of evaluations, progress notes, and plans in line with policies and regulations.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
