Job summary

Northwoods Rehab is seeking a licensed Speech-Language Pathologist to provide and direct comprehensive speech therapy services to residents under physician orders. You will assess, plan, implement, document, and communicate treatment outcomes while coordinating with families and the care team.

You will develop individualized treatment plans with short and long-term goals, monitor progress, and adjust interventions as needed to meet resident goals, including discharge planning and staff

Qualifications

• Master’s Degree in Speech-Language Pathology from an accredited program.
• Certificate of Clinical Competency or eligibility for Clinical Fellowship Year.
• Current state licensure or license eligible.
• Assumes responsibility for ongoing continuing education and professional development.

Responsibilities

• Provides and directs speech therapy services to residents, including assessment, treatment, planning and implementation, documentation, and communication.
• Develops and implements treatment plans by identifying a target list with short and long-term goals.
• Coordinates treatment with resident, family and other team members and ensures goals are integrated into daily activities.
• Monitors resident progress and adapts treatment to attain goals in accordance with physician orders.
• Supervises Rehabilitation Techs/Aides, students and volunteers.

Education

Master’s Degree in Speech-Language Pathology

Certificate of Clinical Competency or eligibility for Clinical Fellowship Year

Current state licensure or license eligible
