Job summary

Sigma Health Rehab in Carthage, NY is recruiting a per diem Speech Language Pathologist (SLP) to join our team of therapists. You will administer evaluations, develop treatment plans, monitor progress, document care, and ensure NY licensure compliance.

Qualifications

• Must have completed an accredited SLP program.
• Current New York State SLP license is required.

Responsibilities

• Administer Speech Therapy evaluations and examinations to diagnose residents and develop treatment plans.
• Monitor residents' progress and adjust treatments accordingly.
• Record information on the initial evaluation, treatment, progress, and discharge of clients.
• Observe and assess progress and prepare detailed reports.
• Maintain resident confidentiality and ensure proper handling of confidential information.
• Ensure compliance with relevant laws, regulations or standards.
• Foster cooperative relationships within the healthcare team and contribute to team efforts.
