Job summary

Northwoods Rehab seeks a Speech-Language Pathologist to provide and direct speech therapy services to residents under physician orders. You will perform assessments, treatment planning and documentation, coordinating with the care team to support communication goals.

The ideal candidate holds a Master’s in Speech-Language Pathology, CCC-SLP or CFY eligibility, and current state licensure or licensure eligibility, with a commitment to ongoing professional development.

Qualifications

• Master’s Degree in Speech-Language Pathology from an accredited program.
• CCC-SLP or CFY eligibility as applicable.
• Current state licensure or licensure eligibility.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provides and directs speech therapy services to residents, including assessment, treatment and documentation.
• Develops and implements treatment plans and coordinates with the care team.
• Monitors progress and adapts interventions to achieve goals.
• Ensures compliance with physicians’ orders and organizational policies.
• Documents results of assessments, treatment and discharge planning.
• Supervises Rehab Techs/Aides, students and volunteers.
• Participates in discharge planning and staff education.
