Job summary

Northwoods Rehab seeks a Speech-Language Pathologist to provide and direct speech therapy services to residents in Forest Lake, MN. The role requires a Master’s in Speech-Language Pathology and CCC or CFY eligibility, with current licensure or licensure eligibility.

Responsibilities include assessment, treatment planning, and coordination with physicians and family. The SLP will document progress, adjust plans, and contribute to discharge planning and family education.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• CCC or eligibility for Clinical Fellowship Year.
• Current state licensure or licensure eligible.

Responsibilities

• Provides and directs speech therapy services to residents under physician orders.
• Conducts screenings of residents to determine need for intervention and treatment.
• Develops treatment plans with goals and methods to achieve them.
• Monitors progress and adjusts treatment as indicated to meet goals.
• Documents results and coordinates discharge planning with care team.
