Evaluate speech, language, cognitive, and swallowing disorders
Develop and implement individualized treatment plans
Provide in-home and community-based speech therapy services
Perform reassessments, progress notes, and discharge planning
Educate patients, families, and caregivers
Collaborate with physicians and interdisciplinary teams
Complete documentation in Home Care Home Base (HCHB) EMR
Manage caseload and schedule efficiently
Master’s or Doctorate in Speech-Language Pathology
Active Pennsylvania SLP License
CCC-SLP (ASHA) – Required
Home Health experience – Required
Experience with Home Care Home Base (HCHB) preferred
Valid driver’s license & reliable transportation
Strong communication, organization, and time management skills
