Job summary

Lock Haven Rehabilitation & Senior Center is seeking a dedicated PRN Speech-Language Pathologist (SLP/CCC) to join our in-house rehabilitation team in a Skilled Nursing Facility and Long-Term Care setting. You will work with an interdisciplinary team to support residents' communication, cognition, and swallowing goals.

The role offers flexibility with scheduling, in-house therapy programs, and a collaborative work environment designed to maximize patient outcomes and quality of life.

Qualifications

• Master's Degree in Speech-Language Pathology required.
• Current Pennsylvania SLP license required.
• CCC-SLP certification required.
• Strong communication and interpersonal skills required.
• Commitment to patient-centered care essential.
• SNF/LTC experience preferred, but not required.

Responsibilities

• Evaluate residents with speech, language, cognitive, voice, and swallowing disorders.
• Develop and implement individualized treatment plans.
• Provide skilled therapy services to improve communication, cognition, and swallowing function.
• Educate residents, families, and caregivers regarding treatment goals and progress.
• Complete timely and accurate documentation.
• Collaborate with physicians, nursing staff, and rehabilitation team members.
• Participate in interdisciplinary care planning and discharge recommendations.

Lock Haven Rehabilitation & Senior Center is seeking a dedicated and compassionate PRN Speech-Language Pathologist (SLP/CCC) to join our in-house rehabilitation team. This position offers the opportunity to work with a collaborative interdisciplinary team in a Skilled Nursing and Long-Term Care setting focused on helping residents achieve their highest level of communication, cognition, and swallowing function.
