• Master’s or Doctorate in Speech-Language Pathology.
• Active Pennsylvania SLP License.
• CCC-SLP (ASHA) – Required.
• Home Health experience – Required.
• Experience with Home Care Home Base (HCHB) preferred.

Responsibilities

• Evaluate speech, language, cognitive, and swallowing disorders.
• Develop and implement individualized treatment plans.
• Provide in-home and community-based speech therapy services.
• Perform reassessments, progress notes, and discharge planning.
• Educate patients, families, and caregivers.
• Collaborate with physicians and interdisciplinary teams.
• Complete documentation in Home Care Home Base (HCHB) EMR.
• Manage caseload and schedule efficiently.
