Deer Meadows Home Health is seeking a Speech Language Pathologist (SLP/CCC) to deliver in-home and community-based therapy across Philadelphia County and surrounding areas. The role offers flexible Part-Time and PRN scheduling, with care provided in home, ALF, IL, and personal care settings.
A PA SLP license and CCC-SLP are required, with Home Health experience preferred. Ideal candidates will work independently, collaborate with physicians and a multidisciplinary team, and document care using
Qualifications
- Master’s or Doctorate in Speech-Language Pathology.
- Active Pennsylvania SLP License.
- CCC-SLP (ASHA) – Required.
- Home Health experience – Required.
- Experience with Home Care Home Base (HCHB) preferred.
Responsabilités
- Evaluate speech, language, cognitive, and swallowing disorders.
- Develop and implement individualized treatment plans.
- Provide in-home and community-based speech therapy services.
- Perform reassessments, progress notes, and discharge planning.
- Educate patients, families, and caregivers.
- Collaborate with physicians and interdisciplinary teams.
- Complete documentation in Home Care Home Base (HCHB) EMR.
- Manage caseload and schedule efficiently.
