Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a full-time Speech Language Pathologist to evaluate and treat residents with speech, language, cognitive-communication, and swallowing disorders, aligning care with physician orders and facility policies.

You will supervise CFY clinicians, document evaluations and plans, and collaborate with an interdisciplinary team to support patient progress and post-discharge maintenance.

Qualifications

• Bachelor's degree in Speech Language Pathology required; Master’s or Doctorate preferred.
• Current California SLP license in good standing.
• Proficiency with electronic documentation systems (Casamba, PointClickCare preferred).

Responsibilities

• Evaluate patients within 24 hours or 1 business day of physician referral.
• Develop treatment plans and obtain physician approval.
• Administer treatments per the approved plan of care.
• Supervise CFY clinicians and SLP Assistants in patient care activities.
• Communicate with the interdisciplinary team regarding patient status and progress.
• Document evaluations, daily notes, progress notes, recertifications, and discharge summaries.
• Provide caregiver and staff education for post-discharge maintenance programs.
