Job summary

Kern River Transitional Care Center in Bakersfield, CA is seeking a PRN Speech-Language Pathologist (SLP). The SLP evaluates and treats patients, documents services in the medical record, and communicates with families, physicians, and the care team.

This role includes supervising CFY and SLP assistants, conducting in‑service training, participating in care conferences, and completing evaluations, notes, and discharge planning.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology.
• California SLP license in good standing.
• Experience with Casamba or PointClickCare preferred.

Responsibilities

• Evaluate patients within 24 hours (or 1 business day) of physician referral; document any delays as required.
• Develop and implement effective treatment plans and obtain physician approval for services.
• Treat patients according to the physician’s plan of care.
• Supervise CFY (Clinical Fellows) and SLP assistants in direct care and related activities.
• Communicate regularly with supervisors and interdisciplinary team members regarding patient progress, concerns, and plans.
• Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
• Complete evaluations, daily notes, weekly progress reports, and recertifications per state and payer requirements.
• Discharge planning and coordinating care with other services.
