Job summary

Sierra Valley Rehab Center in Porterville, CA is seeking a Speech Language Pathologist (SLP) to join our interdisciplinary care team. This position focuses on evaluating and treating speech, language, cognitive-communication, and swallowing disorders in a skilled nursing setting.

The ideal candidate holds a Master’s in Speech-Language Pathology, a California SLP license, CCC-SLP is preferred, and prior SNF/post-acute experience.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current and valid California SLP license.
• CCC-SLP certification preferred.
• Experience in a skilled nursing or post-acute setting preferred.
• Strong communication, organizational, and clinical skills.
• CPR certification.

Responsibilities

• Evaluate patients for speech, language, cognitive-communication, and swallowing disorders
• Develop and implement individualized treatment plans in coordination with the care team
• Provide direct therapy and caregiver/staff training as needed
• Maintain accurate documentation, including daily treatment notes, evaluations, progress reports, and discharge summaries
• Participate in care planning meetings and collaborate with nursing and therapy staff
• Ensure compliance with all applicable regulations and facility standards

Pay Range: $62.00-$65.00 per hour
