Job summary

Kern River Transitional Care in Bakersfield, CA is seeking a Speech-Language Pathologist (SLP) to evaluate and treat residents. You’ll communicate with families and the healthcare team, document services, and participate in ongoing staff training.

Responsibilities include rapid evaluations, treatment planning, supervision of CFYs, and active involvement in discharge planning. California licensure and EHR proficiency are required or preferred, with compensation at $70–$75 per hour DOE.

Qualifications

  • Master’s or Doctoral degree in Speech-Language Pathology.
  • Current California SLP license in good standing.
  • Experience with Casamba or PointClickCare is preferred.

Responsibilities

  • Evaluate patients within 24 hours (or 1 business day) of physician referral and document delays.
  • Develop and implement effective treatment plans and obtain physician approval.
  • Treat patients according to the physician’s plan of care.
  • Supervise CFY (Clinical Fellows) and assist in training staff.
  • Communicate progress and plans with supervisors and interdisciplinary teams.
  • Participate in Patient Care Conferences and Rehabilitation Conferences.
  • Prepare evaluations, daily notes, weekly progress reports, and recertifications.
  • Assist in discharge planning and caregiver education.

Speech-Language Pathologist (SLP) – PRNKern River Transitional Care – Bakersfield, CAPay Range: $70 -$75 per hour (DOE)General PurposeThe Speech-Language Pathologist (SLP) evaluates and treats patients, communicates with families, physicians, and other healthcare team members, and maintains accurate documentation of services in the medical record. The SLP also conducts in-services and provides ongoing training for facility staff.Essential DutiesEvaluate patients within 24 hours (or 1 business day) of physician referral; document any delays as required.Develop and implement effective treatment plans and obtain physician approval for services.Treat patients according to the physician’s plan of care.Supervise Speech-Language Pathologist Assistants in direct care and related activities in accordance with the state practice act.Communicate regularly with supervisors and interdisciplinary team members regarding patient progress, concerns, and plans.Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.Conduct in-service training sessions for facility staff.Complete evaluations, daily notes, weekly progress reports, and recertifications per state and payer requirements.Document any treatment changes per policy and procedures.Instruct patients, families, and nursing staff in maintenance and caregiver programs prior to discharge.Participate in discharge planning and interdisciplinary care coordination.Serve as a clinical preceptor for Speech-Language Pathologist Clinical Fellows and student programs.Comply with all applicable state and federal regulations and facility policies.Supervisory ResponsibilitiesSupervise CFY (Clinical Fellows) as applicable.Assist nursing staff in the training of Restorative Aides.QualificationsEducation & ExperienceMaster’s or Doctoral degree in Speech-Language Pathology.Licensed as a Speech-Language Pathologist in the State of California.Proficiency in Casamba, PointClickCare, or similar EHR systems preferred.Certificates, Licenses, and RegistrationsCurrent California SLP license in good standing.Must meet continuing education requirements as set forth by state and federal guidelines.Physical RequirementsFrequent standing, walking, pushing, and pulling.Occasional sitting, kneeling, and crouching.Must be able to lift up to 50 pounds frequently and assist in transferring patients.Visual and auditory acuity required for accurate assessment and communication.Work EnvironmentLow to moderate noise levels typical of a healthcare setting.Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.Join our dedicated team at Kern River Transitional Care Center, where compassionate, evidence-based rehabilitation helps residents regain independence and improve quality of life.Apply today and make a difference in the lives of our residents every day!We are an equal opportunity employer.
