Job summary

Fort Wayne Health Campus is seeking a Speech-Language Pathologist to evaluate residents’ speech, language, cognitive, and swallowing needs and to develop individualized treatment plans. You will provide therapy aligned with physician orders and care plans while tracking progress and adjusting goals with residents, families, and the care team.

You will join care conferences and interdisciplinary meetings, contributing to the overall care strategy.

Qualifications

• Master’s degree in Speech-Language Pathology is required.
• Current state licensure as a Speech-Language Pathologist is required.
• CCC-SLP credential preferred.
• CPR certification preferred.
• Experience in skilled nursing, rehabilitation, or assisted living preferred.

Responsibilities

• Evaluate residents for speech, language, cognitive, and swallowing needs.
• Build and carry out individualized treatment plans.
• Provide therapy in line with physician orders and care plans.
• Track and document resident progress.
• Coach residents, families, and caregivers on goals and home exercises.
• Work with nursing, therapy, and physicians as part of the care team.
• Join care conferences and interdisciplinary meetings as needed.
