Job summary

Creative Solutions in Healthcare, Inc. is seeking a dedicated Speech-Language Pathologist to join our in-house team serving residents with person-centered care to achieve positive outcomes.

As a Full-Time SLP, you will evaluate swallowing and cognitive deficits, develop individualized treatment plans, and educate residents, families, and caregivers while adhering to regulatory standards. Qualifications

• Active/Valid Texas license in Speech-Language Pathology.
• Knowledge of SNF/Long term care environment preferred.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to improve independence.
• Provide AAC systems and communication supports as needed.
• Educate residents, families, and caregivers.
• Coordinate with other professionals for diagnostic procedures as ordered.
• Maintain accurate billing records and clinical documentation.
