Job summary

Creative Solutions in Healthcare, Inc. is seeking a full-time Speech-Language Pathologist to join an in-house rehab team serving residents with person-centered care to achieve positive outcomes.

The role involves evaluating residents for swallowing and cognitive deficits, developing individualized treatment plans, providing AAC systems, and educating clients, families, and staff. New grads welcome; flexible schedules and strong corporate support.

Qualifications

• Active/Valid Texas license in Speech-Language Pathology.
• In good standing with regulatory agencies and boards.
• SNF/Long term care experience preferred.
• Knowledge of current treatment practices in speech-language pathology.
• Ability to manage residents with different personalities.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Assist residents in goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled, medically necessary SLP assessments and develop individualized treatment plans.
• Provide AAC systems for severe expressive/language disorders.
• Train clients, family, caregivers and other professionals.
• Coordinate with physicians for diagnostic swallowing procedures (MBSS, FEES).
• Demonstrate professional standards of practice.
• Document changes in residents' conditions and maintain accurate billing records.
• Respond to family inquiries in a timely manner.
• Collaborate with Restorative Nursing and care staff.
